Physical Examinations

Students registering for Athletics must have a current physical examination uploaded on the Athletic Team Registration site before the student is authorized to participate in practice. To ensure clearance for participation, physicals must be completed and uploaded online prior to the first day of practice. Paper forms of physicals and alternate year cards will not be accepted, it must be uploaded. 

Physicals taken after April 1st are valid for 2 athletic years from the date of the last exam. If the physical was completed last year, please fill out the Alternate Year portion during registration.
